Machine crash

Hello All,
I have a job was crash on horizontal machine (88E Makino)  due to the Z axis was not retract far enough
so the tool  hit to the Tombstone during 'B' axis rotation, So I have to show my Boss  the Vericut with Machine Simulation and everything  was  clear ,
and no tool crash with the Vericut
I was wondering why Machine Sim did not catched? 
Any help would be appreciated.

Control settings+
Rotary tab
Output intermediate points = YES
